Intakeful, Inc. provides behavioral health intake triage software designed for outpatient group practices and community mental health settings. Our Services include automated intake forms, clinical screening instrument administration (PHQ-9, GAD-7, Columbia Protocol, AUDIT-C, PCL-5), triage routing, insurance pre-verification, and EHR integration. Intakeful is designed to support administrative intake workflows and does not provide clinical diagnosis, treatment recommendations, or clinical care.

3. Business Associate Agreement

If you are a HIPAA-covered entity using our Services to process protected health information (PHI), a Business Associate Agreement (BAA) is required. A BAA is available on all paid plans. Please contact us at [email protected] to execute a BAA before processing any PHI through our Services.

4. Use of the Website and Services

You may use our Website and Services for lawful purposes only. You agree not to:

• Use the Services in any way that violates applicable laws or regulations, including HIPAA and 42 CFR Part 2
• Transmit any unsolicited or unauthorized advertising or promotional material
• Attempt to gain unauthorized access to any part of the Website or Services
• Interfere with the proper working of the Website or Services
• Use our Services to process patient information without a valid BAA in place

5. Clinical Tool Disclaimer

The clinical screening tools available through Intakeful (including PHQ-9, GAD-7, Columbia Protocol, AUDIT-C, and PCL-5) are validated public-domain or freely available instruments. Intakeful administers these instruments as part of an intake workflow. Screening scores and triage assignments generated by our Services are administrative outputs intended to assist clinical staff — they are not clinical diagnoses and do not replace clinical judgment. Your practice's licensed clinical staff are responsible for all clinical decisions.

6. Intellectual Property

The Website and its original content, features, and functionality are owned by Intakeful, Inc. and are protected by applicable intellectual property laws. The clinical screening instruments administered through our Services are owned by their respective rights holders and are used in accordance with their published terms.
